Symptom narrative

Pt received the vaccine Pentacel in error instead of Quadracel that was needed for their age/Vaccination requirement. Verified with DOH that patient does not need repeated Dtap/IPV d/t the timing of the vaccine being out of age range recommendation for vaccine.
