On 04-Mar-2025, the patient was cut by a glass of a broken finger flange of a Pneumococcal Vaccine, Polyvalent (23-valent) (PNEUMOVAX 23) (lot #X027348, expiration date: 24-Jan-2026) 0.5 mL (Skin laceration) syringe (Injury associated with device). The syringe was broken into 2 or more pieces (Syringe issue). The injury occurred after removing the syringe from the tray, while trying to attach the needle to the syringe. The syringe tray was intact and undamaged on receipt. The product was exposed to patient's bodily fluids (Accidental exposure to product). The vaccine was not administered.
