Symptom narrative

The LPN added 1.1 ml of sodium chloride to Covid Pfizer 5y-11y single use vial. LPN administered 0.3 ml of reconstituted Covid vaccine. Infectious disease department was contacted by MD. ID states the dose is invalid and will need to be repeated if parent chooses.
