Symptom narrative

Patient came in stating he needed to receive the first dose of shingrix. After the dose was administered, the patient continued to talk to a staff member who discovered he believed he was supposed to receive the shinrix vaccine yearly. Upon looking at state registry patient had received doses on 6/27/2021, 11/26/22, 5/8/2023 and 3/27/2025.
