Symptom narrative

Pt came in and requested to get second dose of Shingrix vaccine to complete the series, she doesn't know when was the first dose given. I double checked on pt's clinical service but did not check STC. Shingrix was given and pt requested to print out all vaccine records and it showed she already completed the series in 2023 and also it showed she got 3rd dose after that and the dose today was 4th. I went thru with patient on potential adverse reaction and let us know if she experiences. Pt understood and stated that she got many shots and did not keep track, now she had the report of all the vaccines to keep track of.
