Symptom narrative

Itching and redness at injection site started today at 3pm and reported immediately to pharmacist. Patient applied hydrocortisone cream at injection site and that has minimized the itching. Patient did indicate she has been using fragranced soaps-- applied an exfoliator as well as a new lotion at the same site of reaction.
