Product preparation error

Symptom narrative

Vaccine was diluted with the office stock diluet, Merck Sharp and Dohme LLC vaccines diluent, insted of the vial that came in the box in error. Manager was notified, lead CMA, vaccine coordinator, health department, ordering provider, MCIR, and an ERS report was submitted. Per the health department, Menveo was not fully administered. Since the fourth stereotype was not administered the vaccine must be readmisnistered without a waiting period with no harm to the patient. I called and infromed patients mother of this. I informed her that I would be required to do continued education as well as peer review prior to vaccine administrations. She verbalized understanding with no questions.
