Symptom narrative

Child was given an incorrect vaccine on 3/31/25. Nurse thought she was giving Pediarix, ActHIB & Prevnar 20. Nurse was rushed by the parent due to needing to make a phone call & had another appt. scheduled at 4pm. Nurse administered the vaccines at 2:25pm. Nurse failed to scan the vaccines into the computer system prior to administration due to the circumstance of being rushed by the parent & child along with sibling were very uncooperative & unruly during the entire visit making the nurse nurse & frazzled. Nurse informed nursing supervisor of the event to begin the Incident Report process.
