Per pt report "Got Flu vaccine at pharmacy at the end of Jan and has been having pain in her left upper arm since then. Hurts more when trying to raise left arm and when sleeping." Pt was seen at Center for complaint above. Pt was assessed by provider and ordered to receive Physical Therapy on the arm as well as high dose NSAIDS (Meloxicam). Pt to begin PT on 04/02/2025
