Symptom narrative

Nurse drew up the dilutant and administered it to the patient. The nurse never took the dilutant and reconstituted the vaccine. The patient did not receive the actual vaccine during his visit. There is no evidence to suggest an adverse reaction, but the patient did get an injection without receiving the intended medication.
