04/23/2021 12:55 PM S/P #1 COVID Vaccine Moderna the patient became pale, diaphoretic dizzy, felt hot & cold- BP 80/56 P 56 PO 100%, Juice, Pretzels and H2O given, legs raised on another chair. 110 PM c/o feeling better continued HOPM BP 110/74 P 74 - remained alert oriented, did not eat today c/o slight headache, 120 PM color improved, c/c feeling better evaluated by EMS, signed "AMA" discharged ambulatory, wife will drive HX- no medical HX, No Meds, No Allergies
