Symptom narrative

Grandmother of child reached out to staff nurse and reports the childs left thigh was swollen with raised hard area noted on thigh at injection site. Nurse who administered vaccine reports patient moved leg back despite efforts to try and mobilize leg by grandfather when vaccine was administered. When patient moved leg serum appeared to have been injected subcutaneously due to movement with a small amount of serum running down thigh. Called state vaccination nurse who advised report and no need for repeat of vaccination. Small scratch area noted at injection site due to needle pulling out of skin when leg moved.
