Pt was given Kinrix injection 3/31/25 @ 1705. Pt has been given all additives of Kinrix but never as a DTap/IPV combination. Mother states pt had redness at injection site approximately 4 hours post injection but no edema/pain/itching/SOB. When pt woke up in am mother noted increased redness and swelling at the site. Pt returned to clinic 4/2/25. Pt had 4cm x 4cm area of superficial redness and edema noted. No hardness noted under injection site. Pt did not have any respiratory complaints. Denied pain or itching at site currently but did previously. Pt was given Tylenol but no additional medications given. Pt seen by NP and mother was advised to give Benadryl and Benadryl cream as needed. Mother states she and her other children have similar reactions to injections. Pt was advised to return to clinic as needed and follow up with pediatrician 4/4/25. Mother voiced understanding.
