Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: Dizziness / Lightheadness-Severe, Systemic: Fainting / Unresponsive-Severe, Additional Details: Patient received the first dose of gardasil 9 vaccine. Immediately after, she expressed that she doesn't feel good and stayed seated for observation. After approximately 3 minutes, she began to lose consciousness and EMS was called. Her partner was there trying to keep her awake and she did not lose consciousness completely. While EMS was en route, she regained consciousness and was checked out by EMS when they arrived. She chose not to go with them to the hospital and left with her partner.
