Symptom narrative

On 03/27/2025, Patient presented to clinic requesting to get vaccinations that are now due. Pt was to get in total 8 vaccines - Tdap, Hep A, Hep B, Gardasil 9, MMR, Varicella, Meningococcal, Polio. This writer in an attempt to reduce the number of injections, offered a combination vaccine of Proquad with education. However, this writer failed to follow Protocol and confirm patient's eligibility for the Proquad. No adverse reaction reported by patient or Parent.
