Fever, then on 10/9/24collapsed to floor unable to move, blurry/double vision, unable to speak, struggling to swallow, struggling to breathe, rushed to ER and then flight for life to a larger hospital?s Neuro unit. Hospitalized for 10 days, discharged. And readmitted in November 2024 for 5 days because still in crisis. Several months of severe symptoms. Still not recovered. Transitioning from short term disability to long term disability.

09/23/2024 appointment. Vaccines: varicella, hep, flu, MMR? 09/24/24: COVID-19 Pfizer (COMIRNATY), 09/27: 103* fever 9/30/24 10/1/24: part sick days 10/02/24 first day of full sick leave 10/8: eye doc thought I had MG - contacted pcp 10/8: went to quick care, they thought I had COVID, started to feel better and was thinking about going to work the next day. 10/9: I called pcp in AM, they said they can?t see me for a few days. Was feeling ok in the morning but decided to take one more day off to rest. 10/9/2024 Collapsed to floor, rushed to ER, med flight: plasma exchange x5 (over several days with days off) Mestinon 3x/day, steroids 60mg, some treatment for pneumonia, blood clot. Couldn?t walk in assisted, sitting up caused fatigue, difficulty breathing. Plasma exchange was painful and didn?t provide significant improvement. Memory issues throughout visit. After 5x plex, my thyroid levels were extremely low but dismissed it. PCP said wait and see. Very sick for 6-8 weeks. 10/19/24: Discharged from, collapsed in parking lot. Couldn?t make it home and had to stay the night halfway. Memory issues continued. Heart rate increases, gets weird, when Mestinon wears off Extreme constipation, less sensation in genitals 10/23: started PT, stil having difficulty walking and sitting up for long periods. ~ 50 stea day. 10/24/24: PCP discovered pneumonia was back, prescribed Cefpodoxime Proxetil? 10/28: started OT 10/30: neurology 11/4: started counseling, chest X-ray 11/7: went to neurologist appointment and was readmitted 5 days, started IVIG, increased prednisone and Mestinon, started cellcept, started bactrim 11/12: PCP checkin In following weeks, notices slightly more weakness, less sensation, poorer coordination and reactions in left side. 11/18: EMG, single fiber EMG 11/21: visit virtual. A1C low. Physical strength getting better but vision getting worse at this point. 12/2: pcp checkin 12/5: Dr. virtual 12/6: antidepressants Evisit genetic test 12/10: Dr. lung check 12/12: PFT walk test 12/14, 12/15: IVIG at hospital 12/16 swallow study, labs with high GAD65 12/17: Dr. Opthamology 12/18: pcp check in ~¼: started Cymbalta, stopped eliquis . Muscle pain reduced. 1/6/25: began ST 1/7/25: brain, cervical spine MRI 1/17/25 met in person Complaints: confusion/brain fog. Short term memory issues. Loss of sensation in genitals. Neck weakness persists. 1/20, 1/21: IVIG at home, bad reaction 1/23: dietician 1/27: MRI(or ultrasound?) thyroid. Lab result with diabetic blood sugar. Large rbc. High wbc. New labs: blood sugar and thyroid ok, but possible heart and kidney issues? 2/10: neuro opthamologist 2/17: endocrinologist 2/19- 2/26 neurology 3/11: neuro Opthamology Eyes, fatigue, headache, brain fog, heart and lunch?s, weakness still issues. Difficult time knowing where I am in space. Numbness. Genitals. Short term disability ends 4/13
