Symptom narrative

Site: Bruising at Injection Site-Medium, Site: Pain at Injection Site-Severe, Site: Redness at Injection Site-Medium, Site: Swelling at Injection Site-Medium, Error: Shoulder Joint Injury (prolonged pain, tingling, etc.)-, Additional Details: I could see when administering the Arexvy vaccine today on 9/27/24 that the patient still had a bruise on their arm from the vaccine given on 9/13/24. From the location of the bruise, I could tell that the vaccine was given way to high on the arm and when questioned further the patient told me about how much pain they were in for almost 7 days after receiving the vaccine.
