Symptom narrative

Site: Bruising at Injection Site-Medium, Site: Pain at Injection Site-Severe, Site: Redness at Injection Site-Medium, Site: Swelling at Injection Site-Medium, Error: Shoulder Joint Injury (prolonged pain, tingling, etc.)-, Additional Details: Patient has a bruise from a vaccine administered 2 weeks ago and some slight pain when lifting the arm. I was able to see the bruise when giving the Arevxy vaccine to them today on 09/27/24 and when asked, they said the location of the bruise was where the shot was given and it is way too high on the arm to be correct. The patient also said it was extremely painful for close to the full week to lift the arm at all.
