Symptom narrative

Site: Pain at Injection Site-Medium, Error: Shoulder Joint Injury (prolonged pain, tingling, etc.)-, Additional Details: Pt unable to lift arm fully and has shoulder pain s/p 5 months following covid vaccine. She visited doctor who said vaccine must have been given too high. He ordered physical therapy for her. Pt says dr did not examine her, Doctor said he has seen many patients with similar adverse effect from covid vaccine. He advised patient to get all future vaccines in glute. Dr diagosed as SIRVA.
