Symptom narrative

Patient was unknowingly given extra dose of Shingrix as it was not indicated in his chart and patient did not remember getting it previously at pharmacy. Patient experienced fever over night. Resolved by afternoon. Patient's spouse called in to report this noting patient has TBI and therefore suffers poor memory.
