Symptom narrative

On 3/11/2025-1 day after immunization was given patient had swelling at injection site and localized redness and warmth this has persisted since vaccine was given on 3/10/2025. Patient presented to the clinic on 4/3/2025 for further evaluation of the swelling with overlying redness of the skin. On exam patient had an approximate 4 cm x 4 cm x 1.5 to 2 cm deep to skin area of swelling with overlying erythema. Denied fever, chills, shortness of breath or other infectious symptoms. Unable to rule out infection so doxycycline 100 mg twice daily was started and an ultrasound of the area of swelling was ordered along with a CBC and a CRP.
