Symptom narrative

Patient was being seen by NP at the time and consented to a Hep A+B immunization at her visit. Immunization was verified by provider and was given the okay before being administered to patient. After being administered the medical assistant was imputing the immunization into the system and noticed a flag was shown stating that the vaccine was expired. Medical assistant notified Director of nursing and provider and then was advised to notify pt and report accident.
