Symptom narrative

Mother reports over phone call client quickly developed an intractable headache after immunization. Patient went to the ER and was treated at released. Mother reports no history of headaches, she was in a car accident a week before this. She was given fluids, medications of : fentanyl, Benadryl, and Zofran, and had negative CT of the head and neck. Symptoms did not improve till 3/15/2025.
