Symptom narrative

Error occurred when dilution of vaccine for ages 5-11 years with 1.1mL of Sodium Chloride 0.9%. Injected patient with 0.3 mL of solution. Injection was the second covid immunization after recieving first dose of covid vaccine 5 week and 5 days prior when patient was of 4 years of age. No adverse reaction.
