Symptom narrative

Noticed raised lump on RT arm at 9pm, severe headache started at that time, took NSAID without relief, arm pain continued through night, 48 hours after injection RT are redness about 4 inches with large, raised area in the middle. Head ached continues, but less intense.

Levothyroxine, Estrogen, Progesterone, calcium, Vit D3, Claritin

Allergies

Medication: Codeine, levofloxacin, propofol, Cipro, Dilaudid, Lortab, Prednisone ophthalmic solution
