Resident received vaccination to right upper arm via IM injection on 3/31, and redness assessed on 4/2/25 with tenderness and warmth upon palpation. Redness demarcated with alert charting initiated. On 4/3/25 redness had spread beyond demarcation line and increase in pain and warmth reported by patient.
