Symptom narrative

Patient reported redness and swelling in left upper arm, lack of appetite for 2 days, beginning 6 days after administration of vaccine (3/28/25). She went to her PCP on 4/4/25 and was prescribed Augmentin 875 mg twice daily for cellulitis. She reported the adverse reaction on 4/5/25 to the pharmacy when she came to pick up her prescription.
