Symptom narrative

Patient developed left sided chest pain and tingling that developed day of administration. She also developed a welt at the site of administration the next day. She took benadryl daily for the rash however it continued to enlarge. She developed fatigue and chills. She presented to my office 4/7/25 and was found to have a cellulitis on the left arm for which she was prescribed antibiotics.

B complex supplement, baclofen, CoQ-10, fluticasone nasal spray, gabapentin, inderal, keppra, lacosamide, nortiptyline, nurtec, ondansetron, Vit D
