Child was seen in another clinic on March 25, 2025 and was given the DTaP-Hep B-IPV vaccination. The Hep B was missed when checking vaccines needed. There are 4 children in the family. Two were given Hep B at the appointment and two were not. Child was given a Hep B vaccine 8 days after the first one.
