Symptom narrative

Patient described having a headache for a couple of days starting the day after vaccination, sore arm starting during the day of vaccination, and starting 2 days after vaccination, having a sharp pain in back and ribs. He tried taking OTC Tylenol and/or aspirin with some relief, but pain continues. He requested and his doctor prescribed him a muscle relaxant.
