Symptom narrative

Patient began experiencing sweating and stated he felt "weird". Checked blood sugar and contacted clinic. Stated that it was just the body making antibodies. On 9/29/22, patient was found deceased. Autopsy showed enlarged heart which was not previously detected prior to vaccine on 9/2/22 (patient was due to have elective surgery on 10/6 and had been cleared with all pre-op testing).

Current illness

No, patient was healthy at time of immunization. In fact, cleared via labs, chest xray and EKG for elective surgery.

Diabetes, HTN

Metformin, Atacand, Ozempic

Autospy performed on 10/1/2022 shows Acute Cardiac Death.
