Symptom narrative

Patient has called multiple times in regards to having a bad reaction to a vaccine. There are multiple rx notes on file for the patient from multiple pharmacists that have talked to the patient in the past. I called personally and spoke to the patient the evening of 4/3/2025. She reiterated everything that we have on file. She only wanted a regular flu shot, but stated she was pressured or told that she couldn't and in her words, forced to get a high dose flu shot by the pharmacist on file for this complaint. She then stated immediately after the vaccine that she began to have migraines and that her left arm began to lose function and that her daily life wasn't the same anymore. She could no longer do activities that she enjoyed or basic daily activities such as getting ready. She now stated that she went to a orthopedic surgeon and they diagnosed her with SIRVA.
