Symptom narrative

Monday, 4/14/2025 evening Patient went to emergency room for vomiting and low blood pressure. Patient was released that evening and emergency room diagnosed patient with "syncopal episode." Patient returned to emergency room on Wednesday, 4/9/2025 with chest pain. Patient was released from emergency room and referred to cardiologist.
