Symptom narrative

Lot number U8115AA. Adacel given around 2 pm. Next afternoon noted hive on abdomen. Awoke the next day with diffuse hives sparing mouth palms and soles. No other symptoms

URI one month prior to vaccination

Medical history

Type 2 DM, HTN, HLP, Iron defic anemia, environmental allergic rhinitis, Microalbuminuria, acquired hypothyroidism

Other medications

Amlodipine 5mg daily, lisinopril 40 mg daily, ferrous sulfate 66 mg daily, MV 1 daily, levothyroxine 25 mcg daily, rosuvastatin 20 mg daily, metformin 500 mg BID
