She reports that when she received her pneumonia vax she feels like her bone was hit, she pulled her arm back and then the vax was put back in 1 inch below and the rest was injected. Pt then lost range of motion in the arm - she had NP in Cardiology where she works look at it - they are afraid the injection got into the Bursa. There is bruising, a hematoma, and she thinks the capsule is broken. Pt left work to go home and put ice on her arm as she can not move it - Advised pt go to ED for evaluation as we would be unable to treat this in the office at this time. Pt declined ED evaluation - scheduled an office visit with her PCP for the next morning, pt accepted. Pt seen next day in PCP office 4/10.
