Patient and Mom came to clinic for dental care and agreed for patient to have HPV vaccine while they were here. We pulled a report and report showing patient had NOT received HPV vaccine. Gave Mom the VIS for HPV, discussed process, administered HPV vaccine per PHN protocol, monitored patient for 15 minutes, patient tolerated well. They started to leave and Mom asked if patient needed any other vaccines, I advised her we don't show her having the Tdap vaccine. Mom then states patient has lived elsewhere. Mom gets on her phone and pulls up a shot record. She showed me her phone, first vaccine on list was Tdap, so I asked Mom to email the record to me and we will update our records. Mom and patient left. I received the email an hour later, and I reviewed it and it showed patient had already received both doses of HPV vaccine.
