Symptom narrative

Patient came into facility for MMR vaccine. Review of his forecast done with a fellow staff, reviewed that he could receive Zoster and was up to date on PCV. Misinterpreted the communication and thought she had said that he needed the PCV vaccine. Patient last received the PCV vaccine in December 2024. Patient was notified. Medical director notified as well.
