Symptom narrative

Vaccine was administered by pinching the skin and not the recommended z-track method causing some of the vaccine to come back out of the patients arm. It is unknow the amount of vaccine the patient received. CDC was contacted and advised to re-vaccinate the patient and that patient has been informed.

Intractable migraine with aura with status migraineurs, hyperlipidemia, cervical dystonia, back muscle spasms, hypermobility syndrome, myofascial pain dysfunction syndrome, neck muscle spasm, other chronic pain
