Symptom narrative

Per patient, skin on her nose started to peel off 2 days after the vaccines, then bumps developed on arms and hands. Finally, welts showed up on her neck and they've caused lots of itchiness, especially at night.

Chronic kidney disease (stage 4), Essential (primary) hypertension, Hyperglyceridemia, Hyperparathyroidism , Diabetes

Trulicity 1.5 mg once weekly, Amlodipine 10 mg, Losartan 50 mg, Sodium bicarbonate 650 mg, Calcitriol 0.25 mcg, Fenofibrate 145 mg, Metoprolol tartrate 50 mg
