On 28-Mar-2025, the patient was vaccinated with and improperly storage dose of Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD)(rHA), (lot #Y014436, expiration date: 22-Feb-2026), the vaccine was reconstituted with sterile diluent (MERCK STERILE DILUENT) (Dose, route, and anatomical location were not reported) for prophylaxis It was reported that the vaccines had a temperature excursion of 14.92F during a time frame of 1 hour and 40 minutes.
