Symptom narrative

After 4 of the 5 vaccines were given, patient stated she felt a little dizzy and began to turn pale, she passed out for about 15 seconds, and was aroused by calling her name, once she came back too, we provided her with juice X2, a pack of crackers and a bottle of water. Monitored her for 30 mins. Her blood pressure was 107/67 with a HR of 47. Patient was feeling back to self upon departure.
