Symptom narrative

Pt reports initial soreness immediately after the injection that lasted a few days with associated swelling/erythema extending down the deltoid muscle. This improved over the course of 2 weeks , but then developed a throbbing pain extending from the deltoid muscle into the elbow and down the dorsum of her left hand. Pain can range from 0-5/10 and is worst at rest, slightly better with raising arm above head. Difficulty with sleeping secondary to the pain.

Medical history

hyperprolactinemia, pituitary adenoma, anxiety, right hip pain, rhinitis, hypothyroidism, chronic pain of right wrist, h/o hashimoto's thyroiditis, bunion of left foot.

Vitamin C 1000, aspirin 81mg, vitamin D 1,000 IU, escitalopram 10mg, fluocinolone 0.01% shampoo, fluticasone propionate 50mcg/act nasal solution, glucose-MSM-vitamin D-Mn-Ginger, hydrocortiosne-acetic acid 1-2% ear drop solution, ketoconazo
