Symptom narrative

Received a triage call from patient regarding side effects. Per pt she began to have pain, swelling, and redness at the injection site the same day injection was given. The redness and swelling worsened on Sunday 04/13/2025. Per pt, the pain is currently mild at a 5/10 pain. She stated that she only feels the pain at the injection site, denies radiation, says that the swelling is going down and improving. However, she states that she continues to have swelling and mild pain on the right arm where the injection site is after more than a week of receiving the vaccine. Pt will be seeing her provider in the clinic to be assessed in person on 04/18/2025.
