patient a Men B (Bexsero) vaccine in the same deltoid as a MCV4 (Menquadfi) at 11:30am on 4/15 patient had no adverse effects. later in the day I realized I had given both in the same site and knew this was an error so after speaking with provider and immunization program coordinator called and informed patient about adverse effects. I discussed the incident with the Primary Care Immunization Program Coordinator regarding rationale for administering in separate injection sites and have a plan to prevent future incidents.
