Symptom narrative

Incorrect vaccine was administered to pt. Pt received Pentacel vaccine instead of the intended DTaP vaccine. Health department was contacted and they had no major health concerns for the pt, with the only concerns being slightly increased chance of fever and localized reaction. Pt was then notified of the error and what to do if any of the symptoms occurred.
