Symptom narrative

Site: Swelling at Injection Site-Medium, Additional Details: As per letter that was provided by the patient's spouse on 4/15/25, the patient at the time, back in December, reported serious pain at the site where the vaccine was given. Patient went to the local emergency department on 12/22/24, according to the letter provided to us, and that the swelling and pain is ongoing. Also according to the letter, recent aspiration and lab test revealed "yeast/fungus infection" and is "under treatment for the fungal infection."
