Symptom narrative

Site: Pain at Injection Site-Severe, Additional Details: Patient requesting 2 vaccines from a list she had from the doctor. Patient needed Hep B vaccine but was given Tdap. Patient had already received Tdap at the doctor's office so this was a second administration of the vaccine. Patient was given refund and did not have any side effects of vaccine while in store. Patient later reported vaccine error and reported going to the ER due to bad reaction.
