Symptom narrative

1/16/25 pt received flu vaccine. On 4/1/25 pt was here for a visit and complained of right shoulder pain, nerve pain since flu shot was given. pt took Motrin which did not help

abdominal pain, cough, itchy throat

HTN, DM, chronic pain, OA, IDA

Other medications

Arimidex, cymbalta, hyzaar, motrin, priolosec, prenatal vitamin crestor, ozempic, flonas

Allergies

lisinopril, codeine

Lab data

Ultrasound of right upper extremity was ordered and completed on 4/8/25 showed 1)supraspinatus tendinosis 2)mild thickening with fluid distention of the subacromial subdeltoid bursa, possible bursitis 3)fluid distention of the long head biceps tendon sheath, possible tenosynovitis
