PATIENT REPORTED TO NURSE ON 4/30/25 THAT SHE DEVELOPED RINGING IN BILATERAL EARS POST HPV INJECTION ON 4/30/24. PATIENT REPORTED SHE RECEIVED THE HPV INJECTION AGAIN ON 7/8/24 AND 2/5/25. PATIENT REPORTED SHE STILL HAS RINGING IN HER BILATERAL EARS. RINGING IN EARS WAS NOT REPORTED TO NURSING STAFF PRIOR TO DOSE 2 AND DOSE 3 . PATIENT REPORTED SHE HAS NOT FOLLOWED UP WITH A MEDICAL DOCTOR REGARDING THE RINGING IN HER EARS.
