On 13-MAR-2025, the patient was inadvertently vaccinated with Varicella Virus Vaccine Live (Oka-Merck) (VARIVAX) solution for injection, 0.5 ml, subcutaneously in left thigh, lot # Y015557, expiration date 16-SEP-2026 (dose number was not provided) and Measles, Mumps, Rubella and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) solution for injection, 0.5 ml, subcutaneously in left thigh, lot # Y016874, expiration date 07-APR-2026 (dose number not provided), at the same office visit, for prophylaxis (Accidental overdose). Both vaccines were reconstituted with sterile diluent (MERCK STERILE DILUENT) (lot # and expiration date not provided). No symptoms and no additional adverse events were reported.
